The 2519 Missouri form serves as a crucial document for individuals seeking proof of payment related to motor vehicle titles and registrations. Designed specifically for the Missouri Department of Revenue, this form facilitates the request for a receipt that confirms either the payment of title fees or the purchase of vehicle registration. It is important to note that this receipt is not a title itself but rather a verification of payment. Individuals may find themselves needing this form for various reasons, including instances where the original title or registration has been lost, stolen, mutilated, or destroyed. To complete the application, the owner or joint owner of the vehicle must provide detailed information, including their legal name, address, and vehicle specifics such as the make, model, and vehicle identification number. The form must be filled out in its entirety and signed in the presence of a notary public. A fee is associated with the issuance of the title or registration receipt, which must be submitted along with the completed form to the Motor Vehicle Bureau. This process ensures that all transactions are documented accurately and helps maintain the integrity of vehicle ownership records in Missouri.
